Zone-Based Premiums: Why the Same Plan Costs More in Your City
Insurers price health cover by zone, and on many plans the zone follows you into the claim as a co-payment. What each zone means differs by insurer.

Zone rules differ plan by plan even within one insurer's range. Every figure here is from the named plan's own wording and does not transfer to another plan.
Insurers price health insurance by zone, which is a tier they assign to your city or state based on what treatment costs there. Metros sit in the dearest tier and smaller towns in cheaper ones, so the same plan with the same sum insured for the same 35-year-old costs meaningfully more in Mumbai than in Indore.
The part that costs people money is not the premium. It is that on most plans the zone follows you into the claim.
What is a zone in health insurance?
A zone is a pricing tier set by the insurer, normally fixed from the proposer's residential address at the time of proposal. It is not a standard, not set by the regulator, and not comparable across insurers. Each company draws its own map and gives the tiers its own names.
New India Assurance splits the whole country into two. National Insurance uses three. ICICI Lombard uses four, lettered A to D. Bajaj Allianz uses letters. So a Zone 2 policy from one insurer and a Zone 2 policy from another are not describing the same thing, and comparing them by name tells you nothing until you have read both definitions.
Your zone is normally fixed for the policy year and can be changed only at renewal.
The part that catches people: the zone follows you
On most zone-rated plans, buying a cheaper zone and then being treated in a costlier one means you bear a percentage of the claim. This is not a penalty for moving house. It applies to a single admission in a dearer city, which is exactly what happens when somebody travels to a metro for a serious planned procedure.

Read the top row and the bottom row together. On one plan, being treated in Delhi on a small-town premium costs you 28% of the bill. On another, the zone only ever made the premium cheaper.
What 28% actually means
On a ₹5 lakh claim, a 28% zone co-payment is ₹1.4 lakh out of your own pocket, on a policy that is otherwise paying in full. A 20% co-payment on the same claim is ₹1 lakh.
Set that against the saving. A cheaper zone typically takes 15% to 25% off the premium, which on a ₹20,000 policy is ₹3,000 to ₹5,000 a year. At those figures it takes 20 to 47 claim-free years in the cheaper zone to fund a single metro admission's co-payment: ₹1 lakh at ₹5,000 saved a year is 20 years, and ₹1.4 lakh at ₹3,000 a year is 47. The co-payment, meanwhile, lands on precisely the admission that was serious enough to travel for.
That is the trade in plain terms: a certain small saving every year against an uncertain large cost at the worst moment.
Which zone should you buy?
Buy the zone you would realistically be treated in, not simply the one you live in. For most people those are the same and there is nothing to decide. The decision matters for two groups.
If you live in a smaller town but would travel to Delhi, Mumbai or Bengaluru for major surgery, price the higher zone before you buy. The premium difference is usually far smaller than the co-payment you are agreeing to.
If your policy is already on a cheaper zone and you now expect metro treatment, ask about upgrading at renewal. You normally cannot change zone mid-term.
The third option is to sidestep the mechanic entirely. Plans that rate by zone without any zone co-payment, such as ICICI Lombard Elevate and Health AdvantEdge, or Bajaj Allianz My Health Care Plan, let you take a smaller-town premium without carrying the claim-time risk. That is worth a comparison rather than an assumption, because those plans differ on plenty of other terms.
How zone stacks with everything else
A zone co-payment is not the only deduction on a claim. It stacks with whatever else your policy applies: an age-based co-payment, a voluntary co-payment taken for a lower premium, a room-rent proportionate deduction if you took a room above your category, and any sub-limit on the procedure.
On Bajaj Health Guard Platinum, for instance, a voluntary co-payment of 10% or 20% explicitly stacks on top of any zone co-payment already in play. Two 20% deductions on one claim are not 20%.

On the plans in nyvo's policy database as read in September 2026, ICICI Lombard Elevate and Health AdvantEdge price by zone with no zone co-payment, and Bajaj Allianz My Health Care Plan treats zone purely as a premium discount of 15% in Zone B and 25% in Zone C.
Does the zone co-payment apply in an emergency?
Usually not for an accident. New India Mediclaim and ManipalCigna ProHealth Prime Protect both exempt emergency accident admissions, and ICICI Lombard Golden Shield exempts road traffic accidents. A non-accident emergency is generally not exempt, so read your own wording rather than assuming.
