Optima Secure Plus or Ultimate Care: which should you buy?

For most buyers, HDFC ERGO Optima Secure Plus: it doubles your cover from day one and adds 100% of your base cover every year at no extra cost, and HDFC ERGO has the stronger claim record. Care Health Ultimate Care is the one to pick if you need a maternity option, cover below ₹10 lakh, or its Premium Freeze, which holds your base premium at your entry age until your first paid claim.

HDFC ERGO settles 97.8% of claims and draws 8.3 complaints per 10,000 claims, while Care Health settles 96.0% and draws 42.9 (IRDAI data, a weighted average of the last three financial years, latest available as of March 2026). Those figures are published per insurer, not per plan.

Some buyers search for Ultimate Care as Care Ultimate. It is the same plan, and Ultimate Care is the name on its policy documents. Premiums depend on your age, city and cover. nyvo's premium database covers Optima Secure Plus, at about ₹12,800 a year for a 30-year-old with ₹10 lakh of cover in Zone 1 (720 quotes, April 2026), but not Ultimate Care. What do Optima Secure Plus and Ultimate Care have in common?

Optima Secure Plus and Ultimate Care share the features that decide most hospital bills. Neither caps the room you can take, neither has a co-payment in the base plan, and neither puts a sub-limit on any illness. Both refill the cover without limit in a policy year, for the same illness or a different one, and both take adults of any age.

  • Any room, no proportionate deduction. Optima Secure Plus pays room rent at actuals; Ultimate Care pays room rent and ICU up to the sum insured. On a plan with a cap, a costlier room cuts the bill proportionately, except medicines, implants and tests.
  • No co-payment and no disease sub-limits in either base plan.
  • Unlimited restoration, for the same illness as well as a different one. On Ultimate Care it applies from the second claim of each policy year, and the paid Booster adds it for the first claim in the life of the policy; on Optima Secure Plus it is there from the first claim.
  • 60 days of costs before admission, day care procedures up to the sum insured, and AYUSH treatment on both.
  • The same three waiting periods: 30 days for any illness, 24 months for a listed set of illnesses and 36 months for pre-existing conditions, the longest the IRDAI (Insurance Products) Regulations, 2024 allow from 1 April 2024. Read every waiting period, explained.

How do Optima Secure Plus and Ultimate Care compare, feature by feature?

Optima Secure Plus and Ultimate Care differ on how the cover grows, what is built in and what costs extra. Optima Secure Plus builds in 2X cover, an uncapped yearly bonus and consumables. Ultimate Care sells those as options or does without, and in return offers a Premium Freeze, a maternity option, smaller cover sizes and a larger welcome discount.

2X cover or Premium Freeze: which matters more?

The 2X cover matters more for most families, and it is our pick, as in the video. Optima Secure Plus doubles your cover from the first day at no extra cost. Ultimate Care's Premium Freeze is an optional benefit that holds your base premium at your entry age only until the first paid hospitalisation claim, so it helps most when you never use the policy.

How the 2X cover works. HDFC ERGO's Secure Benefit adds 100% of your base sum insured from day one, so ₹10 lakh of cover behaves as ₹20 lakh. Its Infinite Benefit then adds another 100% of the base cover after each completed policy year, claims or not. HDFC ERGO's own illustration is a ₹1 crore policy whose two claims in the third policy year draw on ₹1 crore of base cover, ₹1 crore of Secure Benefit and ₹2 crore of Infinite Benefit, plus ₹1 crore of restored cover.

How Premium Freeze works. Under Ultimate Care's prospectus, the insured person's age is locked at the age they first bought the policy, until a claim is paid. After a paid hospitalisation claim, the premium follows your current age at every renewal from then on. Read the conditions in the wording before you choose it:

  • It applies only to the premium for the base benefits, not to the options you add.
  • On a floater policy, a paid claim for any member unlocks the age for everyone on it. On a policy covering each person individually, only the person who claims loses the locked age.
  • Claims paid under some options, such as Maternity Cover, the newborn covers and Assisted Reproductive Treatment, do not end it; a paid hospitalisation claim does.
  • It locks your age, not the premium itself: if Care Health revises the product, the new premium is charged at your locked age.
  • Choosing it rules out the Early Bird discount of up to 10% on renewals from age 40.

Why we pick the 2X cover. A health policy earns its premium in the year you claim. The Secure Benefit gives you more cover in exactly that year. The Premium Freeze saves money only in the years you do not claim, and it switches off on your first paid admission, when your health has just changed and a lower premium would matter most.

Bonus and consumables: what is built in and what costs extra?

On Optima Secure Plus the uncapped yearly bonus and consumables cover are part of the base plan. On Ultimate Care the base bonus stops at 100% of the cover, and both the uncapped Infinity Bonus and the Claim Shield consumables benefit are paid options. Care Health's own brochure says Infinity Bonus is pre-added to its standard quote, at extra premium.

Here is how ₹10 lakh of base cover grows under each plan's stated rules, before any restoration and with the base cover unchanged:

Two things follow from the table. Ultimate Care can match Optima Secure Plus by year three only if you pay for Infinity Bonus every year, and stopping it forfeits everything built up under it. And Optima Secure Plus is ahead in the first two years, when an uncovered gap would hurt most.

Consumables are the gloves, masks, syringes and other items a hospital bills separately, which most plans refuse to pay. They are the most common reason a settled claim still leaves a bill. Optima Secure Plus covers listed non-medical items through its built-in Protect Benefit. Ultimate Care covers them only if you buy Claim Shield. Read why a settled claim still leaves a bill.

Welcome discount or favourable claims discount?

Ultimate Care gives new buyers a welcome discount of up to 30% in year one, 20% in year two and 10% in year three. Optima Secure Plus gives a first-time buyer of health insurance a favourable claims experience discount of up to 21%, and up to 18% at renewal. A cheaper first year on Ultimate Care can narrow by year three, so compare all three years' premiums.

  • Ultimate Care's discounts multiply rather than add, and the total is capped at 45% of the premium (Care Health's prospectus, as recorded in nyvo's policy database). Its other discounts include a tenure discount on multi-year policies, a family discount of 5% or 10% and an NRI discount of up to 15%.
  • Optima Secure Plus adds a lifetime discount of 5% for a first-time buyer when everyone insured is 35 or under at the start; once given, it applies to every renewal. It also offers a 10% family discount on the individual option, 6% for a two-year policy and 8% for three years or longer paid upfront, and 2.5% if you hold another HDFC ERGO retail policy.
  • The Early Bird discount on Ultimate Care, up to 10% off renewals from age 40 if you bought before 36, is not available with the Premium Freeze. You choose one or the other.

Where does Ultimate Care pull ahead?

Ultimate Care suits a buyer who wants a maternity option, cover below ₹10 lakh or the Premium Freeze. It also pays air ambulance up to the sum insured, grows its discount year by year on a multi-year policy, and rewards a long claim-free run with a one-off Loyalty Boost.

  • Smaller cover sizes. Ultimate Care starts at ₹5 lakh. Optima Secure Plus starts at ₹10 lakh and has no ₹5 lakh tier.
  • A tenure discount that grows each year. Both plans can be bought for up to five years paid upfront. Care Health's discount rises from 7.5% on the second year to 15% on the fifth; HDFC ERGO's is 6% for two years and 8% for three or more.
  • Maternity Cover as an option for insured people aged 18 to 45, with newborn options that only work alongside it. On Optima Secure Plus, maternity comes through the Parenthood add-on, which covers two deliveries but not the newborn. Compare both in how maternity waits and limits compare.
  • Air ambulance up to the sum insured. Ultimate Care pays ambulance by any mode, air included, up to the sum insured; Optima Secure Plus pays road ambulance up to the sum insured but caps air ambulance at ₹5 lakh a year.
  • Loyalty Boost: a further 100% of the cover after seven claim-free years in a row, once in the life of the policy.
  • Instant Cover, a paid option that starts cover for seven named conditions, including diabetes and hypertension, from day 31 on your first policy. Optima Secure Plus's ABCD Chronic Care add-on does the same for up to three of asthma, blood pressure, cholesterol and diabetes.

What should you check before buying either plan?

Both plans are sold as a base plan plus options, so read what is switched on in each quote before you compare prices. Ultimate Care's standard quote carries Infinity Bonus as a pre-added paid option, and the two plans' discounts are structured differently, so the same headline premium can buy different things.

Optima Secure Plus

  • It starts at ₹10 lakh. A quote at ₹5 lakh is for a different product, usually the base Optima Secure.
  • Maternity is only through the Parenthood add-on, with a two-year wait and no newborn cover.
  • Routine doctor visits are only covered with the Optima Wellbeing add-on.
  • Air ambulance is capped at ₹5 lakh a policy year.
  • A single claim cannot exceed the base cover, the Secure Benefit and the Infinite Benefit available in that policy year, per HDFC ERGO's brochure.

Ultimate Care

  • Restoration applies from the second claim of each policy year. The paid Unlimited Automatic Recharge Booster adds it only for the first claim in the life of the policy, not the first claim of every year.
  • Post-hospitalisation is 90 days, against Optima Secure Plus's 180, unless you buy the extension.
  • Consumables are not covered without Claim Shield.
  • The Unlimited Sum Insured option switches off the Cumulative Bonus, restoration, Loyalty Boost, Infinity Bonus, Unlimited Care and Tenure Multiplier.
  • Premium Freeze ends with the first paid hospitalisation claim and rules out the Early Bird discount.

Which plan should you choose?

For most buyers, Optima Secure Plus: twice the cover from day one, an uncapped yearly bonus and consumables cover with no add-on, from the insurer with the stronger claim record. Ultimate Care is the better choice for a narrower group, mainly those who need maternity cover, a smaller policy, or the Premium Freeze.

Choose Optima Secure Plus if you want the most cover in the early years, consumables paid without an add-on, 180 days of costs after discharge, or HDFC ERGO's claim and complaint record.

Choose Ultimate Care if you need cover below ₹10 lakh or a maternity option, or expect to stay claim-free and want the Premium Freeze.
