Care Supreme removes the room rent cap Care carries, refills your cover without limit, and triples the post-hospitalisation window. Care starts lower, from ₹1 lakh.

Care Supreme is the better buy for almost everyone, because it has no room rent cap and Care does. That one clause is the likeliest of all the differences here to cut a real claim. Care is still worth buying in one situation: it sells from ₹1 lakh upward, and Care Supreme starts at ₹5 lakh.

What actually separates Care and Care Supreme?

These are not two versions of the same plan. Care is the older, variant-heavy product sold as Care 1 through Care 9. Care Supreme is the modern single plan, and it is better in four specific ways.

The room rent cap, which is the one that costs you money

Care caps you at a single private room on the ₹5 lakh tier and above. Care Supreme has no cap at all: room rent and ICU are both payable up to the sum insured.

This matters more than it sounds. When you take a room above your eligible category, the insurer applies a proportionate deduction to the charges that vary with room category, which includes nursing, operation theatre and the surgeon's and consultants' fees. Under IRDAI's norms on associated medical expenses (IRDAI/HLT/REG/CIR/151/06/2020, 11 June 2020) your medicines, implants and diagnostics are protected, but the surgeon's fee is often the largest single line on a surgical bill, and that one is not.

Restoration, once against unlimited

Care reinstates the full sum insured once in a policy year. Care Supreme carries Unlimited Automatic Recharge, which refills the base sum insured as often as it is exhausted.

The bonus, 10% against 50%

Care adds 10% of the sum insured for each claim-free year, to a ceiling of 50%, and a claim takes 10% back. Care Supreme adds 50% a year to a ceiling of 100%, and it does not shrink when you claim, which makes it a guaranteed bonus rather than a no-claim bonus.

On a ₹10 lakh cover held five claim-free years, Care reaches its ₹5 lakh ceiling and Care Supreme reaches its ₹10 lakh ceiling. The difference that matters is not the size but the direction: claim on Care and the bonus goes backwards.

The follow-up window, which nobody reads until they claim

Care pays 30 days before and 60 days after hospitalisation. Care Supreme pays 60 days before and 180 days after. Cancer, cardiac and orthopaedic recoveries routinely run past sixty days, and the difference lands on the bill you pay at home after discharge rather than the one the hospital sends.

Care Supreme also covers all day care procedures, where Care covers a specified list, and adds domiciliary hospitalisation, road ambulance and unlimited e-consultations, none of which Care's record lists.

Where Care still wins

Three real advantages, and they are not trivial:

  • It starts at ₹1 lakh. Care Supreme starts at ₹5 lakh. If your budget genuinely does not reach ₹5 lakh, this comparison ends here.
  • An annual health check-up is built in, once a policy year for every insured person, including children. On Care Supreme the same check-up exists but is a paid add-on rather than part of the base plan.
  • Maternity is available on the highest Care variants, on a floater, after a 24-month wait: up to ₹1 lakh at ₹50 lakh to ₹75 lakh of cover and up to ₹2 lakh above that. Below a ₹50 lakh sum insured there is nothing at all. Care Supreme does not cover maternity on any variant.

Care also includes a second opinion for major illness, which Care Supreme's record does not list.

What happens on a ₹6 lakh claim with a costlier room?

Take a ₹10 lakh cover on each plan and a surgical admission billing ₹6 lakh, where the room taken is one category above the eligible single private room.

On Care Supreme there is no room rent cap, so no proportionate deduction arises at all. The admissible claim is paid in full, subject to the usual exclusions.

On Care, the proportionate deduction applies to the room-linked charges: room rent, nursing, operation theatre and the surgeon's and consultants' fees. On a surgical bill the surgeon's fee alone is often the largest single line, and the deduction is applied in the ratio of your eligible room rate to the rate you actually paid. Your medicines, implants and diagnostics are paid in full either way, under the same 2020 norm.

The exact rupee figure depends on the hospital's billing split, which is why no honest comparison can give you one number. The direction is not in doubt: on the same bill, in the same room, Care pays less.

Which one is right for you?

A family of four in a metro, buying ₹10 lakh or more. Care Supreme, without hesitation. The room cap, the recharge and the 180-day follow-up window all favour it, and of Care's three advantages only the built-in check-up is likely to matter to you.

Someone who needs cover now and can afford ₹3 lakh. Care is the only one of the two that sells there, and cover you hold beats cover you cannot afford. Go in knowing what the cheap tiers actually carry: on the ₹1 lakh to ₹4 lakh variants the room is capped at 1% of the sum insured a day and ICU at 2%, which on a ₹3 lakh cover is ₹3,000 and ₹6,000 a day against metro private room rates. The cheaper Care 8 and Care 9 variants also cap cataract, knee replacement, hernia, hysterectomy, prostate, kidney stone, cardiac, cancer, renal and fracture treatment. Ask which of Care 1 to Care 9 is on your quote, because a quote that just says "Care" does not tell you what you are buying.

A couple planning a child in the next three years. Neither is a clean answer. Care's maternity sits only on its highest floater variants behind a 24-month wait, and Care Supreme has none. Look outside these two.

What neither plan does well

Both sit on Care Health, and Care Health draws 42.9 complaints per 10,000 claims on IRDAI's three-year average. That is roughly five times HDFC ERGO's 8.3 and more than twice Aditya Birla's 18.2. The settlement ratio of 96.0% looks fine, and complaints are the number that separates insurers, so it deserves weight in the decision. It applies to both plans equally, so it is not a reason to pick one over the other, but it is a reason to compare both against a plan from a quieter insurer.

Consumables are also excluded from the base plan on both, and are sold as a paid benefit. On a real hospital bill that is a meaningful slice.

Ask Care Health directly. A move between two plans at the same insurer is normally handled at renewal with waiting periods carried across, but the terms are the insurer's to set and are not written into either wording.
