Best Critical Illness Insurance Plans and Riders (2026)
Critical illness cover compared: standalone plans, riders on top term plans and add-ons on health plans, with illness lists, waiting and survival periods.
What is the best critical illness insurance?
The best critical illness cover is the one that pays a large enough lump sum, covers the illnesses you are most exposed to, and lasts as long as you need it. You can buy it as a standalone policy, as a rider on a term plan or as an add-on to a health plan. Compare the illness list, the waiting period and the survival period before you compare the premium.
Critical illness insurance pays a fixed amount when a listed illness meets the policy's definition and conditions, whatever the treatment costs. It does not reimburse hospital bills, so it works beside a health policy rather than instead of one; our critical illness vs health insurance guide explains the difference.
Which standalone critical illness plans are worth considering?
Standalone plans suit buyers who want critical illness cover that does not depend on a term plan or a health policy staying in force. These three set out their cover clearly in their prospectus.
| Plan | What it pays for | Cover | Entry age | Waiting and survival periods |
|---|---|---|---|---|
| HDFC ERGO Optima Vital | A lump sum on listed critical illnesses, medical events and surgical procedures | ₹1 lakh to ₹50 lakh; ₹20 lakh maximum for a new policy above 55 | 18 and above, with no cover-ceasing age | 90-day initial wait; 36 months for pre-existing conditions; a 30 or 90-day survival period depending on the condition, and six months of permanent impairment for some |
| Care Health Assure | A lump sum on 20 listed critical illnesses, medical events and surgical procedures, plus a lump sum on accidental death or permanent total disablement | ₹5 lakh to ₹1 crore in the brochure's standard options | 18 to 65 | 90-day wait, except for accidents; no survival period |
| Aditya Birla Activ Secure (Critical Illness Cover) | A lump sum on listed critical illnesses, with up to 64 covered across the product | ₹1 lakh to ₹1 crore on Plans 1 and 2; ₹5 lakh to ₹1 crore on Plan 3 | 5 to 65; 18 for Plan 3 | 90-day initial wait, 180 days for some conditions; 15-day survival period |
Plan terms from each plan's prospectus, brochure or customer information sheet (Optima Vital UIN HDHHLIP21341V022021; Assure UIN RHIHLIP21375V022021; Activ Secure UIN ADIHLIP18076V011718). Insurers refile products, so check the version you are quoted.
The three insurers settle 97.8% (HDFC ERGO), 96.0% (Care Health) and 96.5% (Aditya Birla Health) of claims, and draw 8.3, 42.9 and 18.2 complaints per 10,000 claims respectively (IRDAI data, weighted average of the last three financial years, latest available as of March 2026).
For a larger lump sum, HDFC ERGO also sells my:health Critical Suraksha Plus, with cover from ₹1 lakh to ₹5 crore.
Which term plans offer a critical illness rider?
A critical illness rider adds a lump sum on diagnosis to a term plan you are already buying, on one policy and one premium, but it often runs for a shorter term than the life cover and carries its own caps.
| Term plan | Rider | Illnesses | Waiting and survival periods | Limits |
|---|---|---|---|---|
| HDFC Life Click 2 Protect Supreme Plus | Health Plus | 60 conditions | 90-day wait; 15-day survival period | Runs for up to 15 years |
| Axis Max Life Smart Term Plan Plus | Critical Illness and Disability | 22 or 64 conditions, with or without total and permanent disability | 90-day wait for major conditions, 180 days for minor; 14-day survival period | Capped at ₹1 crore and at the base cover; a minor claim pays the lower of 25% of the cover or ₹5 lakh |
| Tata AIA Sampoorna Raksha Promise | Criticare and Criticare Plus | 40 critical illnesses | 90-day wait; 30-day survival period on Criticare Plus | A five-year benefit term, renewable up to 75 if no claim is made |
| ICICI Pru iProtect Smart Plus | Health Protect Rider | 20 or 60 conditions, fixed at purchase | 90-day wait; 15-day survival period, longer for some conditions | Any illness traceable to a pre-existing disease is excluded |
| Bajaj Life eTouch II | New Critical Illness Benefit | 10, 25 or 60 illnesses | 90-day wait; 14-day survival period | Runs 1 to 30 years, ending by age 80; cannot exceed the base cover |
Rider terms from each rider's brochure or policy document, as recorded in nyvo's policy database.
Axis Max Life also offers Waiver of Premium Plus, which waives future premiums on 11 listed critical illnesses instead of paying a lump sum.
Which health plans offer critical illness as an add-on?
Several health plans sell a critical illness lump sum as an add-on, which keeps both covers on one renewal.
| Health plan | Add-on | What it pays |
|---|---|---|
| Aditya Birla Activ One MAX | Critical Illness Cover | ₹10, 15, 20 or 25 lakh on one of 20 listed critical illnesses, after a 60-day wait and a 15-day survival period |
| Niva Bupa ReAssure 3.0 | Critical Illness | A lump sum of up to ₹3 crore on a listed critical illness |
| ICICI Lombard Elevate | Critical Illness | A lump sum up to the sum insured, capped at ₹50 lakh, for members aged 18 to 50 when the policy is first issued |
| HDFC ERGO Optima Secure Plus | my:health Critical Illness | A lump sum on a listed critical illness |
Add-on terms from each plan's brochure or prospectus, as recorded in nyvo's policy database. Where waiting and survival periods are not shown, confirm them in the add-on's own terms.
Two related add-ons raise the hospital cover for specific illnesses instead of paying a lump sum on diagnosis: Activ One MAX's Cancer Booster adds up to 100% of base cover for cancer of specified severity, and Optima Secure Plus's Serious Illness Booster adds a further 100% of the sum insured for nine specified serious illnesses.
Which route should you choose?
Choose by how long you need the cover and what it has to sit beside.
- A standalone policy if you want cover that continues whatever happens to your term or health plan. HDFC ERGO Optima Vital has no cover-ceasing age.
- A term plan rider if you are buying term cover now and want one policy. Check the rider term: Tata AIA's Criticare runs for five years at a time and stops at 75, while HDFC Life's Health Plus runs for up to 15 years.
- A health plan add-on if you want one renewal for both covers. It renews with the health policy, and some carry entry limits, such as Elevate's age 18 to 50 at first issue.
What should you check before buying critical illness cover?
Six details decide whether a critical illness policy pays out when you need it.
- The illness list and its definitions. Counts range from 10 to 64 across the options on this page, and a longer list helps only if it includes the conditions you are most at risk of.
- The waiting period. Most options here wait 90 days before a diagnosis counts; Activ One MAX's add-on waits 60.
- The survival period. You must survive a set time after diagnosis: none on Care Health Assure, 14 or 15 days on most riders, and 30 or 90 days on HDFC ERGO Optima Vital, which also requires six months of permanent impairment for some conditions.
- How long the cover lasts. Riders often stop before the life cover does, at 75 or 80.
- Partial payouts and caps. Axis Max Life pays a minor claim at the lower of 25% of cover or ₹5 lakh, and several riders cannot exceed the base cover.
- What a claim does to your other cover. Check whether a rider payout reduces the death benefit, and whether the rider ends once it has paid.

FAQs
Is critical illness insurance the same as health insurance?
No. Critical illness insurance pays a fixed lump sum when a listed illness meets the policy's definition, whatever the treatment costs, while health insurance reimburses hospital bills. It works alongside a health policy rather than replacing one.
What is a survival period in critical illness insurance?
It is the time you must survive after diagnosis before the lump sum is paid. On the options in this guide it ranges from none on Care Health Assure, to 14 or 15 days on most term riders, 30 days on Tata AIA's Criticare Plus, and 30 or 90 days on HDFC ERGO Optima Vital, where some conditions also need six months of permanent impairment.
Should I buy a critical illness rider or a separate policy?
A rider is simpler if you are buying a term plan now, but check how long it runs and what it caps: Tata AIA's Criticare has a five-year benefit term, and Axis Max Life's rider is capped at ₹1 crore. A separate policy such as HDFC ERGO Optima Vital, which has no cover-ceasing age, suits cover you want to keep regardless of your other policies.
Does critical illness insurance cover pre-existing conditions?
Usually not straight away. HDFC ERGO Optima Vital applies a 36-month wait to pre-existing conditions, and ICICI Pru's Health Protect Rider excludes any illness traceable to a pre-existing disease. Declare every condition when you apply.
How much critical illness cover should I buy?
There is no single right amount. Buyers usually weigh the income they could lose during treatment and recovery, costs a health policy leaves out such as travel and home care, and loan repayments that continue meanwhile. A nyvo advisor can walk you through those factors against your income and existing cover.
